In this paper current indications for patent foramen ovale (PFO) closure as well as therapeutic problems related to this procedure have been disscused. The establishing of indications for PFO closure is based on the exclusion of other reasons of ischemic stroke and finding of cause and effect relationship of PFO with stroke, which requires careful cardiological and neurological assessment. The procedure for an experienced cardiologist trained in structural procedures is relatively easy. After PFO closure, double antiplatelet therapy for up to 3 months and prophylaxis of infective endocarditis for up to 6 months is required. The presence of arrhythmias, including atrial fibrillation after the PFO closure, requires further investigation.
