Zanik wieloukładowy — algorytm diagnostyczny i możliwości leczenia
Streszczenie
Zanik wieloukładowy jest schorzeniem samoistnym, występującym sporadycznie. Charakteryzuje się szybkim postępem objawów klinicznych i niekorzystnym rokowaniem. Należy do schorzeń
zwyrodnieniowych układu nerwowego z grupy a-synukleinopatii. Obraz kliniczny cechuje występowanie objawów zespołu parkinsonowskiego, móżdżkowego oraz zaburzeń układu autonomicznego o różnym stopniu nasilenia. W diagnostyce, poza stanem klinicznym, ważną rolę odgrywają badania neuroobrazowe oraz ocena czynności układu autonomicznego. Leczenie objawowe jest mało skuteczne. W leczeniu zaleca się podawanie lewodopy, która u ponad 50% pacjentów w początkowym okresie choroby powoduje przemijającą poprawę kliniczną. Obecnie prowadzone są badania nad nowymi sposobami leczenia tego schorzenia.
