Wewnątrzsercowe urządzenia wszczepialne w niewydolności serca

Streszczenie
Wraz z powszechnym dostępem do wewnątrzsercowych urządzeń wszczepialnych zwiększyła się liczba osób z niewydolnością serca (HF), u których urządzenia te zastosowano w profilaktyce pierwotnej lub wtórnej. Postęp technologiczny umożliwił skonstruowanie nowoczesnych wewnątrzsercowych urządzeń wysokoenergetycznych, dzięki którym osiągnięto zakładane cele terapeutyczne. Należy jednak wziąć pod uwagę to, że implantacja urządzenia może się wiązać z wystąpieniem powikłań, których częstość występowania zmienia się zależnie od doświadczenia operatora, wieku pacjenta, stopnia zaawansowania niewydolności wielonarządowej i schorzeń współistniejących pacjenta. Zabieg implantacji kardiowertera-defibrylatora czy resynchronizatora serca w przypadku prawidłowej kwalifikacji chorego i doświadczonego zespołu kardiologów interwencyjnych jest obciążony stosunkowo niewielkim odsetkiem powikłań wczesnych i odległych. Istotnym elementem postępowania z chorym jest dążenie do eliminacji nieadekwatnych interwencji, które wpływają niekorzystnie zarówno na funkcjonowanie pacjenta, jak i na jego jakość życia. Ważna jest edukacja chorego, a także podkreślenie zagrożeń związanych z nadużywaniem alkoholu, leków, uprawianiem zabronionych sportów oraz niestosowaniem się do zaleceń farmakoterapii. Konieczne są optymalne leczenie farmakologiczne HF i schorzeń współistniejących oraz systematyczna kontrola urządzenia, która obecnie jest możliwa tylko w trakcie wizyt stacjonarnych, ale również dzięki zastosowaniu telemonitoringu.
