Optymalna kontrola glikemii w cukrzycy typu 2 — wytyczne i praktyka kliniczna

Streszczenie
Polskie Towarzystwo Diabetologiczne w „Zalecaniach klinicznych dotyczących postępowania u chorych na cukrzycę 2022” wskazuje jako cele leczenia optymalną kontrolę glikemii, lipidogramu, ciśnienia tętniczego oraz masy ciała. W wybrze preparatów przeciwcukrzycowych powinno się uwzględniać wpływ leku na ryzyko sercowo-naczyniowe, przewlekłą chorobę nerek, epizody hipoglikemii, masę ciała oraz możliwości finansowe pacjenta. Leczenie hipoglikemizujące rozpoczyna się od metforminy. W przypadku współistnienia miażdżycowej choroby układu sercowo-naczyniowego, niewydolności serca, przewlekłej choroby nerek lub wysokiego ryzyka sercowo-naczyniowego związanego z innymi licznymi czynnikami ryzyka zaleca się stosowanie inhibitora kotransportera glukozowo-sodowego 2 lub agonisty receptora glukagonopodobnego peptydu-1, które są objęte tylko częściową refundacją. Biorąc pod uwagę cenę leku, preparatami o największej dostępności są pochodne sulfonylomocznika (SU) oraz agoniści receptorów aktywowanych przez proliferatory peroksysomów gamma. Wśród pochodnych SU o zmodyfikowanym uwalnianiu dużym bezpieczeństwem sercowo-naczyniowym oraz relatywnie niskim ryzykiem epizodów hipoglikemii charakteryzuje się gliklazyd.
