Erectile dysfunction following transurethral resection of the prostate following badder outlet obstruction

Abstract

Introduction. Erectile dysfunction after transurethral resection of the prostate (TURP) is reported in 4-40% of patients. The purpose of this study was to evaluate the incidence of erectile dysfunction following TURP in our patients as well as its correlation with the period after TURP, patients age and volumen of the prostate. Fully impotent patients were excluded from the study.
Material and methods. 140 patients after TURP (54-70 years old) were analysed. The patients were divided into two groups: with proper sexual function and with impaired sexual function. For assessment of sexual function men completed IIEF-5 questionnaire before TURP and after 2, 6 and 12 months. Acaording to the prostate volumen patients were divided into two grups: prostate ≥ 40 cm3 and prostate < 40 cm3.
Results. Before TURP erectile dysfunctiom reported 64 patients - 45.7%, mean age 65 years. 76 (54.3%) patients were potent - mean age 58 years. 12 month after TURP full erection reported 57 patients - 40.7%, mean age 58 years, some erectile dysfunction reported 70 patients - 50%, mean age 65 years and fully impotent was 13 patients - 9.3%, mean age 69 years. One year after TURP patients with prostate volumen ≥ 40 cm3 full potent were 33 patients (23.6%), some erectile dysfunction reported 33 patients (23.6%), impotence reported 5 patients (3.6%). Patients with porstate volumen < 40 cm3 reported full erection 25 patients (17.9%), some erectile dysfunction reported 36 patients (25.7%) and impotence reported 8 patients (5.7%). Recurrence of full or partial sexual activity 12 month after TURP concern patients in mean age group of 65 years. Mean age of patients fully impotent was 69 years.
Conclusions. We conclude that TURP moderately affects the sexual functioning of our patients. The age of patients was the important factor which affects recurrence of sexual functions after TURP. The TURP related erectile dysfunction was less prevalent in patients with large adenomas.
