Włóknienie zaotrzewnowe jako rzadka choroba układowa przysparzająca wielu trudności diagnostycznych — opis przypadku i przegląd piśmiennictwa

Abstract
Wstęp: W pracy przedstawiono różnorodne objawy, zalecane metody diagnostyczne i sposoby leczenia
choroby Ormonda.
Opis przypadku: Przedstawiono przypadek 60-letniego pacjenta, który po wielu miesiącach nieswoistych objawów ogólnych zgłosił się po raz pierwszy do szpitala z objawami napadu kolki nerkowej. Wykonano badania obrazowe (USG, CT jamy brzusznej oraz PET), uwidaczniając prawostronne wodonercze z poszerzeniem układu kielichowo-miedniczkowego i moczowodu po stronie prawej oraz obecność tkanki włóknistej w przestrzeni zaotrzewnowej. Na postawie wywiadu, objawów klinicznych i typowego obrazu w badaniach radiologicznych rozpoznano chorobę Ormonda. Zastosowano leczenie polegające na założeniu cewnika DJ do prawego moczowodu oraz skojarzonej terapii immunosupresyjnej metyloprednizolonem i azatiopryną, co doprowadziło do szybkiej remisji choroby.
Wnioski: Choroba Ormonda ma charakter wielodyscyplinarny, a w jej diagnostyce i leczeniu pomocna jest współpraca specjalistów różnych dziedzin medycyny.
