Problemy dializoterapii pacjentów z nefropatią cukrzycową

Abstract
Cukrzyca i mocznica nasilają wzajemnie angiopatię,
neuropatię oraz niedokrwistość i dlatego kontrola cukrzycy
i intensywna dializoterapia są niezbędne dla
poprawy rokowania co do chorobowości i przeżycia
w tej grupie chorych. Nie każdy dializowany chory na
cukrzycę ma nefropatię cukrzycową, ale powinien
być tak traktowany. W ostrej niewydolności nerek
u pacjentów z cukrzycą korzystna jest intensywna
insulinoterapia i utrzymywanie glikemii w granicach
110-140 mg/dl. U dializowanych z powodu przewlekłej
niewydolności nerek leczenie cukrzycy jest
w zasadzie takie jak w okresie przeddializacyjnym,
a zabiegi dializ nie odbiegają jakościowo od stosowanych
u pacjentów bez cukrzycy. Z leków hipoglikemizujących
najbezpieczniejsze są glipizyd i gliklazyd
- krótkodziałające pochodne sulfonylomocznika
drugiej generacji. Dializa otrzewnowa i hemodializa u pacjentów z cukrzycą dają porównywalne wyniki
leczenia. Najpoważniejsze problemy u dializowanych
z cukrzycą to przyspieszona angiopatia (retinopatia,
stopa cukrzycowa) oraz nasilona neuropatia wegetatywna
(skłonność do hipotensji w czasie hemodializ,
gastropareza). Mimo ogromnego postępu w dializoterapii
pacjentów z cukrzycą, jaki się dokonał w ostatnich
latach, rokowanie co do dożycia tej grupy pacjentów
jest nadal gorsze niż u pozostałych dializowanych.
Dalszej poprawy oczekuje się dzięki
stosowaniu agresywnej kontroli cukrzycy, prewencji
jej powikłań (zwłaszcza angiopatii, w tym zespołu
stopy cukrzycowej), unikanie intensywnej ultrafiltracji
i zwiększenie intensywności dializ, a u dializowanych
otrzewnowo stosowanie w płynie dializacyjnym
ikodekstryny zamiast glukozy w wysokim stężeniu.
