Nawrót glomerulopatii po przeszczepieniu nerki

Abstract
Kłębuszkowe zapalenie nerek (KZN) jest przyczyną
schyłkowej niewydolności nerek w około 1/4 przypadków.
Swoistym problemem klinicznym jest
skłonność do nawrotów tej choroby po transplantacji
nerki. Istnieje zróżnicowanie między poszczególnymi
typami KZN w zakresie częstości nawrotów
oraz ich wpływu na rokowanie co do utrzymania
przeszczepu. Najbardziej agresywny i niekorzystny
przebieg kliniczny ma nawrót KZN na podłożu ogniskowego
szkliwienia/stwardnienia kłębuszków. Inną
postacią o niekorzystnym rokowaniu jest mezangio-kapilarne KZN. Dotychczas stosowane formy terapii,
obejmujące nasilenie standardowej immunosupresji,
powtarzane plazmaferezy oraz dodanie do
leczenia rituximabu, są skuteczne jedynie w części
przypadków. Ostatnio opisano zastosowanie u chorych
z ogniskowym szkliwieniem/stwardnieniem
kłębuszków (FSGS) galaktozy w dużych dawkach,
co miałoby zahamować lokalne działanie na podocyty
i obniżać ogólną aktywność czynnika przepuszczalności
białka, krążącego we krwi.
