The case report of 72-year-old patient in the context of the of bipolar disorder stages and form of psychiatric care lased for nearly half a century in Poland

Abstract

The paper presents a case of a patient suffering from bipolar disorder since decades. The work was conducted in the context of presented in the literature stages of progress of the disorder. An attempt was made to answer the question whether the community service unit form of care can be a factor which modifies the course of the disease even after decades of ambulatory and hospital care. Description of the case is based on a detailed interview with the patient and her family. The available medical records of 48 years of treatment in hospital, in outpatient and in community unit were examined to describe presented case. The multi-annual mileage was modulated by forms of patient’s care. It has been observed that traditional in Poland combination treatment in the ward and outpatient clinic accelerated episodes of illness. Community care inhibited the progression to more severe stages, even at the late stages of the disorder. Most likely, comprehensive community care can be a protective factor against the deterioration in the course of bipolar disorder. Presented description of bipolar disorder may be a clinical confirmation several concepts course of BD, and at the same time deny neuroprogresion’s beliefs about the inevitability of the disease and its clinical manifestation.
