Antiplatelet therapy and urgent surgical procedures. Acute appendicitis two days after stent implantation - a case report

Abstract
Antiplatelet therapy has become a standard primary and secondary prophylaxis regiment in cardiology. Perioperative coronary stent thrombosis is a catastrophic complication that can occur in patients with both bare-metal or drug-eluting stents. The most important reason of this complication is discontinuation of antiplatelet treatment. Non-cardiac surgery increase the risk of stent thrombosis, especially when it is performed early after implantation of stent. Since antiplatelet therapy increases the risk of bleeding but decreases the risk of cardio-vascular events, it is very important to consider whether to stop or to continue this therapy. A case of 60-year-old male patient who underwent abdominal operation 2 days after bare-metal stent implantation is reported. Problems with correct treatment are discussed.
