Korzyści kliniczne i ekonomiczne z zastosowania sekwencjonowania następnej generacji w diagnostyce chorych na niedrobnokomórkowego raka płuca z rzadkimi mutacjami
Streszczenie
Diagnostyka molekularna u chorych na niedrobnokomórkowego raka płuca (NDRP) jest niezbędna do podjęcia decyzji terapeutycznych, zwłaszcza dotyczących terapii celowanych. Obejmuje ona analizę ekspresji PD-L1 oraz obecności mutacji lub rearanżacji w genach EGFR, KRAS, BRAF, ALK, ROS1, NTRK1/2/3 i RET. W Polsce zaleca się przeprowadzenie analiz w kierunku mutacji punktowych w eksonach 18., 19., 20. i 21. genu EGFR oraz rearanżacji genów ALK i ROS1. Z uwagi na czas, koszty oraz dostępność materiału biologicznego coraz częściej wskazuje się na korzyści z rutynowego stosowania NGS u pacjentów z NDRP zamiast wykonania wielu pojedynczych testów. Przeprowadzono analizy farmakoekonomiczne, mające na celu ocenę wpływu zastosowania NGS w praktyce klinicznej na budżet płatnika publicznego w Polsce w porównaniu z obecną praktyką. Wykazano zmniejszenie wydatków inkrementalnych płatnika publicznego związanych z diagnostyką molekularną z użyciem NGS u wszystkich kwalifikujących się do niej chorych o ok. 3,4 mln PLN w 2023 i 2024 roku oraz zmniejszenie kosztów diagnostyki przypadających na jednego chorego o 1695 PLN (21%). W pracy przedstawiono również skuteczność i bezpieczeństwo amiwantamabu u chorych na NDRP z insercją w eksonie 20. genu EGFR. Podsumowując, NGS powinno stanowić preferowaną metodę diagnostyczną u pacjentów z zaawansowanym NDRP.
