Streszczenie
Historia zastosowania immunoterapii w leczeniu chorych na raka nerkowokomórkowego sięga lat 90. XX wieku. Wykorzystywano interferon-alfa oraz interleukinę-2, jednak postępowanie to było obarczone istotną toksycznością przy niezadowalającej efektywności (w przypadku interferonu-alfa). Wprowadzenie do praktyki klinicznej inhibitorów punktów kontrolnych układu immunologicznego (ICIs, immune-check point inibitors) zmieniło zasady postępowania terapeutycznego u chorych na jasnokomórkowego raka nerki. W chorobie uogólnionej wykorzystywane są skojarzenia dwóch ICI, ICI z inhibitorami wielokinazowymi lub ICI w monoterapii. Szczególnie należy podkreślić wysoką aktywność immunoterapii w przypadku raka nerkowokomórkowego z odrożnicowaniem sarkomatycznym. Z uwagi na brak molekularnych czynników predykcyjnych w procesie kwalifikacji do leczenia kardynalne znaczenie ma znajomość specyfiki poszczególnych schematów leczenia w odniesieniu do ich aktywności oraz profilu działań niepożądanych.
Streszczenie
Historia zastosowania immunoterapii w leczeniu chorych na raka nerkowokomórkowego sięga lat 90. XX wieku. Wykorzystywano interferon-alfa oraz interleukinę-2, jednak postępowanie to było obarczone istotną toksycznością przy niezadowalającej efektywności (w przypadku interferonu-alfa). Wprowadzenie do praktyki klinicznej inhibitorów punktów kontrolnych układu immunologicznego (ICIs, immune-check point inibitors) zmieniło zasady postępowania terapeutycznego u chorych na jasnokomórkowego raka nerki. W chorobie uogólnionej wykorzystywane są skojarzenia dwóch ICI, ICI z inhibitorami wielokinazowymi lub ICI w monoterapii. Szczególnie należy podkreślić wysoką aktywność immunoterapii w przypadku raka nerkowokomórkowego z odrożnicowaniem sarkomatycznym. Z uwagi na brak molekularnych czynników predykcyjnych w procesie kwalifikacji do leczenia kardynalne znaczenie ma znajomość specyfiki poszczególnych schematów leczenia w odniesieniu do ich aktywności oraz profilu działań niepożądanych.
