Failure of immune tolerance induction (ITI) in a child with haemophilia A complicated by inhibitor — case report

Abstract

The development of factor VIII inhibitor alloantibodies following haemophilia A treatment is a serious complication. which may explain why the therapy is so difficult and not always successful. In this study we present the case of a 16-year-old with haemophilia A and factor VIII inhibitor primarily diagnosed at the age of 8 months. After three independent attempts at immune tolerance induction (each time the effect was only temporary) he was administered 11 doses of rituximab, but no permanent eradication of the inhibitor was achieved. Consequently, bleeds into joints became the major problem, both for the patient and the doctors. Prophylactic therapy with aPPC is now administered because join bleeds have become frequent.
