Abstract

Only approximately 30% of hypertensive patients will achieve goal blood pressures of < 140/90 mm Hg with monotherapy, regardless of the medication used. Fixed-dose combination therapy with two different classes of antihypertensive agents will achieve goal pressures in most of hypertensive patients. The advantages of combinations include: greater blood pressure decrease than monotherapy, fewer side effects, improved adherence to treatment and possibly lower cost of therapy. Combination of bisoprolol, a modern cardioselective beta-adrenolytic agent, and amlodipine, the most popular dihydropyridine calcium channel antagonist, mixed in one pill demonstrate different mechanisms of hypotensive action, which secures antihypertensive efficacy regardless of patient’s age or plasma renin activity. Such combination seems to be most suitable for young patients with stage 2 and 3 arterial hypertension, women, and patients with hypertension complicated with ischemic heart disease or diabetes.
