Rak tricholemmalny okolicy pachwiny o nieznanej lokalizacji pierwotnej
Streszczenie
Autorzy przedstawiają przypadek 49-letniej pacjentki z rakiem tricholemmalnym o nieznanej lokalizacji pierwotnej. Guz pachwiny, leczony jako przepuklina pachwinowa, stopniowo powiększał się w ciągu 6 miesięcy do średnicy około 6 cm. Pobrano wycinki do badania histopatologicznego, które wykazało zmianę typu raka płaskonabłonkowego. W międzyczasie w obrębie guza doszło do rozwoju owrzodzenia z wyciekiem ropy. Pacjentkę skierowano na badanie tomografii komputerowej (TK), które uwidoczniło powiększone węzły chłonne w pachwinie po stronie guza. Podjęto leczenie chirurgiczne obejmujące wycięcie guza i limfadenektomię wzdłuż naczyń biodrowych. Wykryto raka w obrębie pochewki włosa. Podjęto chemioterapię i radioterapię. Po czterech miesiącach nie stwierdzono oznak wznowy miejscowej ani przerzutów odległych. Autorzy omawiają napotkane trudności diagnostyczne i różne opcje leczenia raka tricholemmalnego.
