Practical approach to the intensification of insulin therapy in type 2 diabetes mellitus - the algorithm of switch from biphasic human insulin to biphasic aspart insulin

Abstract

In recent years, the need of intensifying and improving metabolic control in type 2 diabetes is becoming more and more important. The majority of insulin treated type 2 patients in Poland receives human insulin premixes that in many cases are not sufficient to prevent diabetes complications, particularly macroangiopathy as a consequence of inadequate postprandial glycaemic control. Biphasic insulin analogues seem to be a good tool to improve diabetes control in those patients, they are convenient to administer and provide appropriate control the postprandial glycaemia peaks without compromising hypoglycaemia risk. This paper contains practical considerations and guidelines based on authors clinical experience that could be helpful in safe switching type 2 patients from human insulin premixes to biphasic insulin aspart preparations.
