Changing diabetes - an international project taking into account activities to improve efficacy of type 2 diabetes management. Part II. The Polish program for improving glycaemia control

Abstract

This article is a continuation of the paper published in “Diabetologia Praktyczna” No. 8-9 and describes the Polish project of glycemia control improvement. The next step taken in the International Changing Diabetes Project was establishing local committees in 8 countries including Poland. After evaluation of glycemia control in type 2 diabetic patients, several activities were undertaken to improve the situation. Of a number of proposals the winning educational project of the Polish Society of Diabetology (PSD) was chosen for implementation. The main goal of the project is to improve diabetes control in Poland through more effective realization of PSD recommendations for combined care i.e. provided by both the specialist and family physician. Apart from permanent education of specialists, the project includes a system of partnership support between specialists and family physicians. The program covers the whole territory of Poland and will be realized under the patronage of the PSD, College of Family Physicians and National Specialist in Family Medicine.
