Abstract

Giant free-floating thrombus in brachiocephalic vein is a rare, life-threatening complication in patients with Paget-Schrötter syndrome (PSS). We report a case of a 21-year-old body builder who was admitted to our department with a 6 days history of pain and swelling in the right upper extremity. Color Doppler ultrasonography diagnosed thrombosis of the right subclavian vein with free-floating, giant thrombus in right brachiocephalic vein. Vena cava superior filter deployment followed by catheter-direct thrombolysis and surgical decompression of the subclavian vein with the first rib resection was performed. We present the preoperative work-up and successful treatment of this unusual case, to our knowledge, never reported before.
