Abstract

The aim of this paper was to present case of stroke occuring in the course of systemic lupus erythematosus. Stroke was a initial presentation of SLE in this patient. Women (17 years old) without known stroke risk factors in anamnesis and diagnostic investigations was admitted after episode of loss of consciousness and with left hemiparesis. Magnetic resonance imaging (MRI) of the brain revealed bright intensity focal lesion with right hemisphere aspecially in basal ganglia area. Cerebral angio-CT demonstrated total occlusion of right middle cerebral artery in M1 segment and stenosis of the left anterior cerebral artery. In EEG diffuse slowing was noted. Laboratory studies revealed elevated level of white blood cells (WBC) - 16.200 mm3 and sedimentation rate (SR) 70 mm/h. Anti DNA, antinuclear antibodies (ANA) and IgG anticardiolipin antibodies were positive. LE cells were present. On the basis of sings, clinical and laboratory changes the diagnosis of systemicus lupus erythematosus was made.
Conclusion. In young patient with cerebral ischaemia without stroke risk factors extensive laboratory investigations should be carried out because neurological changes could be the initial symptom of systemic lupus erythematosus.
