Abstract

Both transcranial doppler and/or transcranial color-coded duplex combined with extracranial duplex are excellent fast, noninvasive and bedside methods to diagnose the site and degree of arterial occlusion in patients with acute stroke. By these methods the changes in vessel patency during thrombolytic treatment and in the natural course of the disease can be followed. Recently the protocol of the fast track TCD was developed, allowing very quick diagnosis of the site of arterial occlusion. The grades of residual flow were classified in the Thrombolysis of Brain Ischemia Scale and the TCD symptoms of recanalisation were described. The chances to successful thrombolysis and clinical improvement were determined based on TCD diagnosis and recanalisation monitoring. The first studies were published on the therapeutic use of low frequency ultrasound in assisting the treatment with tissue plasminogen activator. The both methods are easy to use and non-invasive, howevere the thick bone window in some patients and strong dependency of the results on the examiner experience and quality of the equipment limit their use.
