Leukoaraiosis as a subtype of cerebral small-vessel disease

Abstract

Leukoaraiosis (LA) describes an abnormal appearance of cerebral white matter, especially located in the periventricular and deep subcortical areas, seen on neuroimaging. Leukoaraiosis represents the second subtype of cerebral small-vessel disease (CSVD), connected with diffuse arteriopathy of the perforating arteries and hypertension (diffuse-CSVD). In the pathogenesis of LA essential role plays chronic cerebral ischaemia and endothelial dysfunction together with reduced cerebral blood flow and increased blood–brain barrier permeability. Cerebral microangiopathies are currently recognized as a major burden of disease in elderly people in developed countries worldwide. CSVD is responsible for 20-30% of all strokes and is the most common cause of vascular subcortical dementia.
