Drenaż otorbionych zbiorników płynowych pod kontrolą endosonografii — przydatność na podstawie medycyny opartej na faktach
Streszczenie
Wytworzenie okołotrzustkowych zbiorników płynowych (PFC) jest jednym z najczęstszych powikłań ostrego ciężkiego zapalenia trzustki. Większość ostrych okołotrzustkowych zbiorników płynowych zanika samoistnie i nie wymaga drenażu endoskopowego. Znaczna część ostrych martwiczych zbiorników trzustkowych zostaje otorbiona i może wymagać leczenia endoskopowego. Endoskopowy drenaż PFC z implantacją protezy jest obecnie preferowaną metodą drenażu ze względu na mniejszą liczbę powikłań w porównaniu z drenażem chirurgicznym lub przezskórnym. Zastosowanie zarówno protez metalowych, jak i plastikowych w drenażu endoskopowym zbiorników trzustkowych jest skuteczne. Protezy metalowe mogą mieć przewagę w sytuacji, gdy w zbiorniku obecna jest martwica ze względu na możliwość bezpośredniej nekrozektomii endoskopowej przez protezę. Powikłaniami, które mogą wystąpić w związku z drenażem endoskopowym okołotrzustkowych zbiorników płynowych, są: krwawienie, perforacja, posocznica, migracja protezy oraz tak zwany buried LAMS syndrome.
