Renal function recovery in patient treated with hemodialysis for 18 months and renal allotransplant graft

Abstract

Chronic kidney disease is more prevalent among adults and its incidence is steady increasing. Recovery of renal function is not frequently seen among patients suffering from chronic renal failure treated with dialysis and is estimated at 0.3–8%. 57 year old white female was admitted to hospital due to complications of alcohol abuse. During hospitalization the end stage renal disease was diagnosed and treated with hemodialysis for 18 months. After meeting the criteria for kidney transplant patient underwent successful transplantation. Function of the transplanted kidney started straight post reperfusion, postoperative hemodialysis was not needed. 10 months after patient discontinued taking immunosuppressive agents for almost 13 months, but the creatinine concentration remains stable 2.5–3.3 mg/dl. She was hospitalized and diagnostic tests of graft and own kidneys (ultasonography, scintigraphy, pyelography, biopsy of the graft) were performed. Recovery of own renal function and allograft disfunction was found. On the day of discharge from the hospital patient did not need hemodialysis, creatinine level was 2.8 mg/dl. 3 months after, on her last visit in Transplantology Department was found in a very good condition with creatinine 2.5 mg/dl and diuresis 2.5 l. Was referred to nephrologist but she never follow-up. The possibility of recovery of own renal function should be sought and assesed, especially when early kidney transplantation is considered, there should be a place for regular monitoring of own renal function and the indication for the nephrectomy should be carefully considered.
