Kiedy rozpocząć leczenie dializą?

Abstract
W ostatnich latach w Stanach Zjednoczonych oraz
Europie utrzymuje się tendencja do wcześniejszego
rozpoczynania leczenia nerkozastępczego, którą
uzasadnia się dłuższym przeżyciem pacjentów
wcześniej włączanych do dializy. Jednak w świetle
ostatnio opublikowanych badań słuszność tej tendencji
wydaje się dyskusyjna. W niniejszym opracowaniu
zwrócono uwagę między innymi na niedoskonałość
wzoru MDRD w kontekście kwalifikacji do
leczenia nerkozastępczego oraz na zjawisko błędu
z tytułu czasu wykrycia w ocenie przeżycia pacjentów
dializowanych. Decyzja o rozpoczęciu leczenia
dializą powinna być indywidualna i obejmować nie
tylko wartość eGFR, ale również dokładną ocenę
stanu klinicznego pacjenta.
