Zwiększona sztywność tętnic u pacjentów z przewlekłą chorobą nerek

Abstract
Pacjenci z przewlekłą chorobą nerek (CKD, chronic
kidney disease) charakteryzują się wysokim ryzykiem
sercowo-naczyniowym. Jednym z powikłań CKD
w układzie sercowo-naczyniowym jest zwiększona sztywność
tętnic. Mianem sztywności tętnic określa się
zmniejszenie właściwości elastycznych naczyń tętniczych.
Zwiększoną sztywność tętnic wykazano
zarówno pacjentów z CKD w okresie przeddializacyjnym,
jak i leczonych nerkozastępczo dializami lub
przeszczepieniem nerki. Jedną z metod oceny sztywności
tętnic jest pomiar prędkości fali tętna (PWV,
pulse wave velocity). U chorych hemodializowanych
wykazano, że podwyższona PWV ma niekorzystne
znacznie rokownicze. W niniejszym artykule przedstawiono
czynniki związane ze zwiększoną sztywnością
tętnic u pacjentów z CKD leczonych zachowawczo
oraz różnymi sposobami terapii nerkozastępczej.
